I've always gone with cold on the thought that cold causes the blood vessels to constrict. Can identify with not wanting the pack to touch my head though. When I'm into a HA for awhile, I find myself holding the pack a 1/2 inch away from my scalp. Lots a good that does. But a lot of what we do during a HA doesn't make much sense when we look back at it.
