It sounds like he has his mind made up and nothing you say is going to change it. Something may have to happen for him to finally listen to you. You know the Serenity Prayer, change what you can, accept what you can't change, and know the difference. If he keeps up with 3 packs a day that may get him someday. Hate to hear that. Is he any more receptive when he is not in pain, or upset? Best thing is if he would just read some of our post. We are not crazy people, we are hurting people. When I am in my headaches, my jaws hurt so badly you would think all my teeth were rotten. But as soon as my headache leaves, so does the jaw pain. That shows there is a coalation. I used to have to deal with my step father with his heart, and his head was as hard as the pavement. Sometimes, you just have to let them do what they want to do. These men can be pretty stubborn.
