blurred vision before onset of headaches?

Hi,
my brother is in the middle of an episode of what might be cluster headaches. Apparently he can be okay one minute and rolling around in agony the next. This was preceeded, last week, by blurred vision lasting a couple of days, and that went away when the headaches started. The headaches have been bad enough to make him go to the hospital, but they just said it was a sinus infection, and his doctor agrees. He's on antibiotics, which aren't helping, and pain killers, which aren't strong enough.
I haven't seen any references anywhere to blurred vision preceeding the headaches, so I was wondering if anyone here has had that happen?
