The problem with Cafergot is that much of the ergotamine is destroyed in the stomach and so the effective dose is unpredictable.
Might try Ergomar SL (ergotamine also). This is a small tab slipped under the tongue where it quickly dissolves and is absorbed by the bed of blood vessels around the gums. It likely goes into action faster because of this mode of absorption.
If he has sleeping attacks rather regularly, some docs recommend one Ergomar about 30-minutes before going to bed. This often prevents attacks and because ergotamine stays in the body for a number of hours, it may carry him thru the night.
Has he tried DHE nasal spray? It has a decent track record and while more expensive than Ergomar/Cafergot, might be worth a trial if her finds ergotamine helpful.
