Just for another perspective, I'm sort of between the two of you. I certainly don't know 'biofeedback' or RET, but I also don't (usually) beat my head on the wall. I'm with DJ on the light and noise---who gives a f***? I could stand right in front of the speakers at a Grateful Dead concert (if Jerry were still here) and not even notice the music.
I 'withdraw' during a dance with the demon. I pace relentlessly and drive my fingernails into the cartilege of my 'opposite side' ear. But I don't scream, etc. I need to pull back into myself, focus on the spike in my head and kind of stare the demon down. Okay, MF, do your best. BTDT, and we will again. But in an hour or so, you'll be gone, bastard, and I'll still be here.
Finally, I'm with you on the handholding/soft voice stuff, DJ. Thanks for your concern everybody, but please leave me the f**k alone. I can't battle with the demon if you're distracting me.
