It's all too easy for all of us to get very frustrated with doctors and ins. companies, and both get plenty of (often justified) bashing here. But the objective isn't to vilify docs or insurance companies. Vilifying the demon, is of course, perfectly acceptable.
drummer's post, and the earlier one Ueli linked to, are excellent primers on how we can take control of our lives and disorder. I might suggest that we post the names of the insurance companies that we manage to convince to recognize us and our treatments. I imagine it would be helpful if, say stevek and drummer happen to have the same ins. comp. Stevek could then cite drummer as a precedent in his efforts to obtain validation.
My insurance is Aetna's US Healthcare, and I'm in the PPO group. My first script this past cycle was for Imitrex tabs-30. No problem at all. The pharmacist did the typical approval submission thing and received immediate authorization. USH paid the standard (for my plan) 80%. Refills were likewise covered.
I've never tried O2, since I'd never heard of that treatment before coming here, so I can't comment on USH's position on that, but I'll be sure to let you know next cycle.
