First, I'm not a doctor, but I had to take medical terminology when I worked as a hospital admin person. It's possible my answer isn't the total one but here's what it means to me:
In the context of the article, "primary neural discharge" means to me that the initial cause of the headache is in the nervous system, while the pain mechanism itself (dilated blood vessels or whatever) is a secondary effect. "Primary" was taught to me to mean "here's where it started".
If anybody else can flesh this out (or correct me) please do!
