Sound`s to me you got burn out. Yes I think you should speak to your doctor an see if he can help.
Also is there anyone that would come in and help you. This way it take`s some of the pressure off of you. It could be daily or just a couple time`s out of the week. It will give you something to look forward to. In this time you can heva just you time.
