If the responses thus far are any indication, yes, they have all killed themselves.
My episodes started five years ago, at age 58, and they're gotten progressively worse, to the point where I think I'm becoming chronic.
Just yesterday, however, I was talking to a friend whose father-in-law, age 78, had suffered from cluster headaches for years but who, upon having triple bypass surgery, went into what now appears to be complete remission.
That's not much help, I know -- just some more cheerful, optimistic, anecdotal news from the front lines. I might add that the thought of living with these hideous things into my old age really does put thoughts of suicide uppermost in my mind right now.
